Initial Applicants for Accreditation

New centers seeking designation as a MBSAQIP-Accredited Center can apply using the MBSAQIP Quality Portal link above. It is required that all new centers submit an application before gaining access to the Pre-Review Questionnaire (PRQ). 

Please note: Your center must meet all standards and intend to actively pursue accreditation for the designation level sought prior to submitting an application. Please carefully review the MBSAQIP Standards prior to application.

If you have not already done so, please review the Standards before proceeding with your application.

Existing MBSAQIP Centers

MBSAQIP-Accredited Centers are required to submit a renewal PRQ every three years to maintain accreditation.

Documentation from previous accreditation cycles, such as previous Accreditation Reports, are accessible through the historical MBSAQIP Application Portal. Please note: The historical MBSAQIP Application Portal is only available for review of historical documentation. All current actions regarding your MBSAQIP Accreditation will be managed through the new MBSAQIP Quality Portal.

MBSAQIP Reaccreditation Process

The Primary Contact at your center has been issued a username and password for the MBSAQIP Quality Portal. Your center will be notified about the accreditation renewal process six (6) months prior to your center’s accreditation end date. The renewal notice will be sent via e-mail to the Primary Contact with further details regarding the renewal process.

Your center will be assigned to a MBSAQIP Program Coordinator who will be your primary point of contact for the duration of the application process.

International Data Collection Center Applicants

Participation as a Data Collection Center is now available to International Centers.

Please note: Participation as a MBSAQIP Data Collection Center is NOT available to centers within the United States and Canada.

Data Collection Centers can apply using the MBSAQIP Quality Portal. No additional materials beyond the application are required for centers that are only seeking Data Collection participation. The bariatric surgeon leader at your center (MBS Director) is required to view the MBSAQIP Data Collection Webinar prior to applying. All staff or hospital administrators who will be integral to the successful implementation of a Data Collection program are encouraged, although not required, to view the webinar as well.
